Laparoscopic Roux en Y Gastric Bypass
A gastric bypass procedure for weight loss involves several steps. First,
a small stomach or gastric pouch is created by dividing the stomach with
staples. Then the small intestins is divided and one segment (Roux limb)
is brought up and connected to the pouch (gastrojejunostomy) and the other
segment is connected to the Roux limb at a measured distance away from
where the Roux limb is connected to the gastrojejunostomy. Roux-en-Y refers
to the way in which the bypass is performedit looks like a Y
Open Roux en Y Gastric Bypass
The decision for open versus laparoscopic Roux en Y gastric bypass is
made between the patient and the surgeon. Some patients may be offered
the open gastric bypass due to a high BMI or because of previous abdominal
surgery. The safety of the patient is the utmost concern.
Laparoscopic Gastric Banding
Gastric banding has been performed for weight loss since the late 1980s.
This techniques was modified for laparoscopic use and LAP-BANDs
have been placed since 1993 in Europe and other places around the world.
The LAP-BAND is adjustable because it has a silicone balloon which can
be inflated. Inflation of the band is accomplished by instilling sterile
saline into a port that is placed under the skin (See figure). The effect
of inflating the band is to restrict the amount of food a person eats
so weight loss occurs because less food is ingested.
Open/Laparoscopic Duodenal Switch
Depending on the patients BMI and co-morbidities, an open BPD-DS
may be recommended instead of a laparoscopic procedure. It is also possible
that a laparoscopic BPD-DS can not be completed using small incisions
and a larger incision is made. The BPD-DS operation involves removing
part of the stomach and rerouting the intestines to a greater degree than
the gastric bypass procedure
Open/Laparoscopic Biliopancreatic Diversion
Depending on the patient's BMI and co-morbidities, an open BPD may be
recommended instead of laparoscopic procedure. It is also possible that
a laparoscopic BPD cannot be completed and larger incision is required.
BPD or biliopancreatic diversion is also known as the Scopinaro procedure
after the surgeon that originated it.
