Roux-En-Y Gastric Bypass
The Roux-en-Y, RUNY, or RNY is a more complex operation, which evolved
from the VBG to provide more consistent and successful weight loss. The
small upper pouch (1/2 ounce) is completely separated from the distal
stomach which is left in place. A "Y" shaped segment of small
bowel is attached to the pouch with a narrow opening. This delays the
mixing of food and digestive juices from the distal stomach resulting
in poorer absorption of calories and nutrients. This both restricts food
intake and interferes with absorption resulting in more consistent weight
loss, but has a higher risk of complications or side effects. 80% of patients
lose at least half their excess weight with the RNY procedure. The RNY
averages a 3-day hospitalization with a return to full activity in 7-10
days. A liquid protein and soft diet is suggested for 3 weeks post op
Vertical Banded Gastroplasty
The Vertical Banded Gastroplasty (VBG and Laproscopic VBG) limits food
intake by creating a small pouch (1/2 ounce) in the upper stomach with
a narrow outlet (1/2 inch) reinforced by a mesh band to prevent stretching.
The pouch fills quickly and empties slowly with solid food, producing
a feeling of fullness. Over eating results in pain or vomiting. This restricts
food intake. This is the simpler of the two operations, with a generally
lower risk of complications and shorter hospital stay. The disadvantage
of Laparoscopic Vertical Banding - VBG is it usually results in less weight
loss than Laproscopic Roux-en-Y or RNY. It does not restrict intake of
high calorie liquids (sweets) and the pouch can stretch with overeating.
As a result 20% of people do not lose weight and only half of people lose
at least 50% of their excess weight with a Vertical Banding Gastroplasty.
This procedure can usually be performed as a 23 hour procedure with return
to full activity in 7-10 days. A soft diet, with 6 small meals is suggested
for 3 weeks after surgery.
Lap - Band Adjustable Gastric Banding
The LAP-BAND is designed to induce weight loss by restricting food consumption,
and is a variation of the Vertical Banded Gastroplasty (VBG). An adjustable
elastic band is placed around the upper stomach to create a small 15cc
(one half ounce) pouch with a narrowed outlet. The outlet size can be
adjusted by injecting saline into a small reservoir (port) placed under
the skin at the time of surgery, and connected to the band by an I.V.
tubing. The pouch fills quickly with solid food and empties slowly to
relieve hunger and produce a feeling of fullness. Overeating results in
pain or vomiting to limit food intake.
